The aim of the service is to engage, empower and equip our tenants to enable individuals or families to successfully manage their lives and tenancies.
The emphasis of the Support & Wellbeing Coordinator is to encourage engagement with ourselves and to signpost to other appropriate services required to meet their often complex and varied needs. To improve the quality of their lives and successfully maintain their tenancies.
We are liaising and setting up working together protocols with those commissioning Support & Wellbeing in the city. These include Manchester City Council, Early Help Hubs, Health & Social Care Services, Greater Manchester Police, NHS Manchester, employment and community groups.
Referrals can be accepted with the resident's consent to the referral being made from any of the following:

When you click submit the form automatically gets sent to the email@example.com inbox and will be logged and allocated by the Place Manager for Support & Wellbeing.
Tenants can contact us directly to discuss the service, or a member of staff, a relative or carer or other professional can contact on us on the tenant's behalf with their consent.
The referral where possible should be completed and submitted online by email or post. They will then be assessed for need and suitability for the service. If it is an appropriate referral the case will then be allocated.
The Support & Wellbeing Coordinator will firstly arrange to see the tenant to complete a full needs assessment. Following the assessment, a decision will be made as to whether support will be offered. Residents can receive support either within their own home or from a local office.
The team also hold drop-in sessions in four locations:
Monday 9am-12pm at Worsley and Platt Court, Rusholme
Wednesday 9am–12pm at Cundiff Court, Levenshulme
Thursday 1–4pm at The Place, Fallowfield
Friday 9am-12pm at Abbey Court, Gorton
